Special tests Upper extremities: -to determine if patient has a fracture or pain in the shoulder -tell patient to abduct both arms sideways; alternately; 90 degree angle then tell patient to slowly lower the arm of the affected shoulder -positive if fail to slowly lower arm or smoothly lower arm or severe pain and may indicate rotator cuff tear -positive if patient will abruptly lower the arm because of pain
