INTRAHEPATIC Surgical excision is the only potentially curative treatment for cholangiocarcinoma. No surgery if portal invasion, trunk of the hepatic veins invasion, metastasis. Intrahepatic cholangiocarcinomas are treated like hepatocellular carcinoma, with hepatectomy when possible. EXTRAHEPATIC The tumor is unresectable. both hepatic ducts extensively involved (type IV), the main trunk of the portal vein extensively involved metastasis, Bismuth-Corlette type I or II : common bile duct excision with portal lymphadenectomy, cholecystectomy, with bilateral Roux-en-Y hepaticojejunostomies
