Surgery is indicated for Massive hemorrhage with shock. Ongoing transfusion requirements (>6 U packed red blood cells). Recurrent hemorrhage. Surgery: excise ulcer (gastric) or oversew vessel (Gastro-duodenal art)or antrectomy/Billroth II Prognosis: 75% managed by endoscopy alone; surgery required in <20% of cases; 15% mortality from massive hemorrhage
