Most perforated ulcers are located anteriorly. Symptoms and signs: Usually sudden, severe upper abdominal pain. Patient is severely distressed, lying quietly. Rebound tenderness and abdominal rigidity. Abdominal x-ray reveals free intraperitoneal air in 85% of patients . Localized or generalized peritonitis
